These forms are a resource for Officials to complete as prescribed by the NCWLUA:
- Independent Contractor Form – 2026 NCWLUA Independent Contractor Form
All Officials must complete this form and send it to the NCWLUA before an Official can work an NCWLUA event. A completed version of this form is required each year.

- NCWLUA Incident Report – NCWLUA Incident Report
For any situations that have a significant impact on a game, the Lead Official is required to complete and submit an NCWLUA Incident Report. This includes all ejections of a player, coach, or other participant (fan, etc.). If an ejection occurs, the Lead Official will call the assigner ASAP following the game and then send an Incident Report once they arrive home.
